Population Health Alliance Announces Winners of the 2014 Outstanding Leadership in Population Health Awards
Washington, DC (PRWEB) January 23, 2015 -- Population Health Alliance (PHA), a leading non-profit organization dedicated to advancing the principles of population health, recently announced winners of the 2014 “Outstanding Leadership in Population Health Awards.” Recipients from 10 companies and organizations were recognized for contributions to the population health industry, as demonstrated by presentations shared at the 2014 PHA Forum.
The association has presented its widely recognized industry awards since 1999. This is the third year in a row that a panel of health care industry experts selected winners through a juried process.
“The work of the 2014 winners showcases just how far the population health management industry has come,” said Karen Moseley, director of research for PHA. “Through their presentations, we see the value of experience and innovation coming together, setting the stage for future strides in the rapidly evolving field.”
Winners of the 2014 Outstanding Leadership in Population Health Awards included:

Presentation: Creating a Culture of Health Through Organizational Support
Flynn identified the key elements of organizational support, best practices, and how these are best measured for effectiveness; and described key research findings and case studies that support building a culture of health.
Paul Mulhausen, MD, MHS, FACP Chief Medical Office, Telligen, and Mike Herndon, DP, Senior Medical Director, Oklahoma Health Care Authority
Presentation: Effective Patient and Provider Engagement: Integrating the Medical Home into the Coordinated Neighborhood
Attendees at this presentation learned how to incorporate the member/provider engagement model into their business practices, as well as how to begin bending the cost curve with improved health and financial outcomes.
