Purple Binder Joins Healthify Team to Support Evolution of Social Determinants Space

("Purple Binder"), providers of technology and services for community health, announced the completion of a transaction for Purple Binder to join Healthify. This event will create the largest network of social determinants of health (SDoH) providers and care coordinators, covering over 7.5 million Medicare and Medicaid patients, in the US.
Healthify was founded in 2013 by community health workers who witnessed, first hand, how closely poor health outcomes and the increasing cost of care were tied to unmet social needs such as food insecurity and affordable housing. Healthify sought to offer a better way for healthcare organizations to coordinate around social services and connect their beneficiaries with the services they needed to improve the quality of care and reduce healthcare costs.
Today, Healthify helps payers and providers manage the unaddressed social needs of their most vulnerable populations. The company offers a web-based platform that supports a broad spectrum of activities within care coordination; including identifying SDOH concerns, finding community resources and services, tracking social needs, and coordinating referrals. The addition of Purple Binder to the Healthify team will enable Healthify to extend its services to a larger population of people in need.
