Qualis Health Named Award Finalist for Care Transitions Work
Seattle, WA (PRWEB) February 27, 2015 -- Qualis Health, one of the nation’s leading population health management organizations, has been named a finalist in the 6th Annual Case In Point Platinum Awards in recognition of outstanding commitment toward continuous improvement in care transitions. The nomination acknowledges Qualis Health’s success in reducing hospital readmission rates through its Communities for Safer Transitions of Care program.

Case in Point is recognizing Qualis Health’s three-year effort (August 2011 through July 2014) supporting 19 communities in Idaho and Washington to convene broad coalitions focused on reducing unnecessary hospital readmissions. Each of these communities included one or more hospitals and other providers such as home health agencies, nursing homes, physician practices and hospices. The initiative was associated with avoidance of over 7,600 30-day hospital readmissions and estimated Medicare savings of over $88 million.
Qualis Health led its care transitions program as the Medicare Quality Improvement Organization (QIO) for Idaho and Washington, under contract to the Centers for Medicare & Medicaid Services. The QIO Program is among the largest federal programs dedicated to improving health quality at the community level.
Winners of this year’s Case in Point Platinum Awards, which recognize organizations that have positively impacted America’s healthcare delivery system through innovative healthcare management programs, will be announced May 7 in Baltimore, MD.
