Quality Center for Rehabilitation in Lebanon Partners with TwelveStone
Murfreesboro, Tennessee (PRWEB) August 29, 2017 -- Quality Center for Rehabilitation & Healing in Lebanon has partnered with TwelveStone Health Partners, introducing the center’s signature, state-of-the-art Pulmonary Independence Center (PIC). The Pulmonary Independence Center is the first joint program for TwelveStone Health and will serve patients with respiratory ailments, assisting individuals in healing faster and stronger. The highly-skilled specialty program is in conjunction with respiratory care provider, Encore Healthcare.
The PIC program will specialize in pulmonary disease management including diagnoses of congestive heart failure, COPD, and pneumonia. Through disease specific protocols, the clinical team at Quality Center will support patients through symptom management, pulmonary exercises, and therapeutic education.
Quality Center, under the leadership of administrator, Adrienne Pintilie, will dedicate ten patient beds to the PIC unit. The Center will acquire a full-time respiratory therapist that will support the program, including after hours on-call guidance. Quality Center will also feature onsite and remote patient monitoring through telemedicine. As the joint venture’s first site for Encore’s widely-respected PIC program, Quality Center will have the resources to treat and support patients with chronic cardiopulmonary illnesses through patient and staff education, advanced therapies, and complex care coordination, including remote patient monitoring. This will reduce costs for providers, and dramatically improve patient clinical outcomes.
“The rise of bundled payments, together with readmissions penalties, have hospitals motivated to move patients to rehabilitation facilities that truly understand the dynamics of respiratory care for chronic conditions such as COPD,” says Shane Reeves, president and chief executive officer of TwelveStone. “Encore’s PIC joined with our own skill in transitions of care provides Quality Center for Rehabilitation and Healing, a skilled nursing facility in Lebanon, with a niche program offering cardiopulmonary care that few others can match.”
“Skilled nursing and rehabilitation facilities like Quality Center meet a need for patients on devices such as noninvasive ventilators or high flow therapy, as well as patients with tracheostomies who need to transition to breathing on their own,” says Zach Gantt, president and chief executive officer of Encore. “We get them back to living independently, and historically have helped reduce hospital readmission rates by 60 percent.”
Quality Center for Rehabilitation and Healing in Lebanon is a member of the CareRite Centers Network, a dynamic network of 26 skilled nursing and rehabilitation centers across Ohio, Pennsylvania, New York, New Jersey, Florida, and Tennessee. Quality Center’s mission is to provide unprecedented levels of genuine care and customer service for their patients in a soothing and tranquil healing environment. The uncommon denominator at Quality Center is that every touchpoint of every experience will not only meet, but will exceed your expectations.
