SPAC international provides ONC HIT certified Chronic Care Management Physician, Patient and Pharmacy Portals for physicians to deliver Chronic Care Management to their Medicare patients with two or more chronic diseases lasting for 12 months or longer. SPAC International achieved this certification right after Medicare released the CCM code 99490 back on the Halloween night of 2014. SPAC is enrolling physicians nationwide into it's Chronic Care Management Cloud TM and patients are provided mobile Chronic Care Management TM applications for their smart phones for Drug Adherence TM and Medication Therapy Monitoring TM via the clinical teams that work with each patient. The smart phone applications have received the US Food and Drug Administration (FDA) approval.
