South Country Health Alliance Deepens Commitment to Quality Healthcare for its Medicaid Community by Selecting Casenet TruCare

TruCare was selected for its flexible configuration and mobile capabilities, as well as the ability to interoperate with other systems. The SCHA health information exchange integration with the TruCare care management platform enables SCHA care managers to have a complete view of all member information, resulting in better care for SCHA members.
SCHA is a county-owned health plan providing healthcare coverage in 12 Minnesota counties. The health plan offers five programs to meet the healthcare needs of Minnesota Health Care Program enrollees in its service area.
The launch of SCHA’s care program was a planned strategic move over a two-year period to bring complex care management in-house with newly hired care managers and to identify a long-term technology partner that could support their strategic direction. SCHA selected Casenet as that partner because the flexibility and interoperability of TruCare will provide their care managers with a complete picture of each member and the ability to intervene more quickly and be more active in the delivery of care across the entire care continuum. This is especially important for their high acuity patients.

The SCHA health information exchange will be integrated with TruCare to get real-time notifications for hospital admissions, discharges and transfers (ADT). Through professional services provided by Casenet partner, TESCHGlobal, TruCare’s web services API will be used to integrate HL7 ADT message data. By enabling dynamic, multidisciplinary care teams to better coordinate, collaborate and communicate, TruCare decreases the time to intervention and increases efficiency. This will result in improved SCHA member engagement and outcomes as well as reduced costs.
With a single platform for utilization, case and disease management, TruCare delivers unmatched flexibility to implement, coordinate and manage clinical, wellness and quality programs and enables providers, care managers, families and members to successfully deliver integrated care management. SCHA will take advantage of not only the TruCare platform but also TruCare Mobile®. TruCare Mobile is a tablet application that will enable SCHA care managers to efficiently assess their members when in the field and securely access clinical, care management and member information.
