Seattle, WA (PRWEB) March 21, 2017 -- Navigating Cancer, the leader in oncology patient-engagement technology, is excited to introduce telehealth and remote monitoring capabilities to its Care Management solution. Together, these capabilities improve medication adherence, help reduce the negative impact of side effects and provide patient insights during chemotherapy treatment.
Patients undergoing chemotherapy are often faced with complex drug regimens and serious side effects that can negatively impact treatment effectiveness, outcomes and cost. With the right tools, however, patients can gain a clear understanding of their treatment plan, participate in shared-decision making with their care team and get help right when they need it.
Care Management combines comprehensive triage software that prioritizes patient issues by severity of risk, symptom management pathways that enable consistent, high quality management of acute patient needs and Health Tracker, a remote monitoring program that provides medication reminders and a convenient way keep clinics informed of side effects when they occur. The benefits of this type of highly coordinated and comprehensive patient care results in faster interventions, fewer hospitalizations, and a personalized patient experience.
This solution not only allows the care team to maximize their efficiency and provide proactive patient care, but has been built with the patient experience in mind. When a patient is enrolled into Health Tracker, a check-in schedule can be setup to align with their treatment plan, which can depend on factors like their oral chemotherapy cycle or the pattern of adverse side effects that are likely to occur. Reminder schedules can also be setup to simply check-in with the patient every week or two, according to the patient’s preferences. Text messages are sent according to these settings and prompt the patient to report back on medication adherence and side effects.
These patient-reported outcomes and any incoming triage calls are surfaced in the clinic’s triage dashboard where all of the information is stratified according to risk and severity. This enables triage nurses to monitor the highest risk patients and provide quick intervention. When assessing patients, the symptom management pathways enable nurses to perform a thorough evaluation and determine the best course of action in an efficient and effective way. Following clinical prompts and accompanying branching logic, the nurse is empowered to use their best clinical judgement to not only deliver immediate care but also set patient follow-ups, thereby closing the care loop.
“Telehealth give patients a lifeline to get help when they need it” said Navigating Cancer CEO Gena Cook. “Combined with remote monitoring, the solution empowers the care team to get ahead of issues before they are too severe to manage outside of the hospital.”
