Welch Heathcare and Retirement Group's Hancock Park Rehabilitation and Nursing Center in MA Bridges Care Settings with New Program
Quincy MA (PRWEB) November 24, 2015 -- As older adults transition across multiple care settings the need for increased communication between each care provider is crucial to ensuring quality healthcare and reducing avoidable hospital re-admissions. Nursing staff from Hancock Park Rehabilitation and Nursing Center in Quincy, MA, in partnership with Milton Hospital and the VNA Care Network and Hospice, recently completed a program co-sponsored by the Massachusetts Senior Care Foundation and Western Massachusetts Nursing Collaborative (WMNC) called Care Transitions Education Project (CTEP).
CTEP is an innovative training approach designed to increase nurses’ knowledge and skills necessary for effective patient-care transitions. Nurses from Hancock Park, along with nurses from Milton Hospital and VNA Care Network & Hospice, received 32 hours of training, which included interactive and independent learning activities.
“Our nurses are always striving to deliver the very best care they can,” says Melanie Brace, RN, director of nursing services at Hancock Park Rehabilitation and Nursing Center. “The CTEP program helped connect all the moving pieces – creating a deeper degree of understanding and respect between the different care settings,” adds Brace. Equally important, states Brace, was the tremendous insight gained from Hancock Park nursing staff’s personal observation of nurses providing care in other settings. “This had a positive impact on how our nurses approach the process of patient admissions and discharges,” say Brace.
According to the Massachusetts Senior Care Association, the CTEP Project is designed to achieve the following objectives:
- Increase competency of practicing nurses and nursing students to lead and improve care transitions
- Increase mutual respect among practicing nurses across care settings
- Improve cross continuum coordination and collaboration among care team members
- Demonstrate nurse-led quality improvement in care transition practice and work processes
